Modified MacIntosh-Micheli ACL reconstruction. The modified MacIntosh-Micheli ACLR involves harvesting an approximately 2cm (~50%) x 15cm long segment of the ITB that is left attached at Gerdy’s tubercle [1]. The graft is then passed “over-the-top” and through the inter-condylar notch before being tunneled beneath the inter meniscal ligament and secured to the periosteum proximal tibia variably with sutures or anchors.
