1) A pdf of the survey instrument used
2) An Excel spreadsheet containing the 188 completed responses to the survey. To maintain participant privacy some answers have been removed or redacted. Country level data has also been replaced by continent level data. Details of anonymisation steps are in the README tab of the Excel spreadsheet.
