most controversial pathologic aspect in inflammatory bowel disease is
the presence of dysplasia. The definition, historical evolution,
subtypes and grading systems need a clear understanding of the biology
and morphology of the lesions.
The biological process is similar regardless of the type of inflammatory bowel disease and comprises widespread genetic abnormalities, chromosomal instability, and activation of the genetic crisis checkpoint under TP53 control. The multifocality is the rule, and the presence of non-adenomatous dysplasia is a strong predictor of synchronous or metachronous invasive malignancy. In contrast, adenomatous dysplasia shows no significant differences in this conditions in sporadic neoplastic transformation.
