Here's the CV I used when applying for an E-Science Librarian position with Indiana University Bloomington in August 2011. I was later hired on in December 2011.

I'm making this available to others so they can see more examples of library CVs that are appropriately edited and reformatted to suit the requirements of particular jobs. (There are surprisingly few online!)
